Application Form?

My neighbor and her husband will be at the July 22nd taping and want to audition. I searched the website and couldn't find the application form. Does anyone know where it is?

I don't think it's on the website but here are the questions that were on it when I took the test in June.:


WHO WANTS TO BE A MILLIONAIRE
AUDITION APPLICATION

And they handed you another question at later auditions, about charities. If they were to match your winnings for a charity of your choice, which one would it be and do you have a personal connection to it?
I just ordered chicken and an egg from Amazon. I'll let you know.

Thanks, all.

My neighbor lost her mother (unexpectedly) in 2007 and her father this January -- the same month she lost her job.

I'm really hoping to give her some confidence and boost her spirits! And, of course, I'll recommend she join the bored!

Good luck to your neighbor.

I lost my mother this spring and my father-in-law this past August. It is not easy - here's hoping she is doing well.

BTW, if anyone is interested in my responses to the questionnaire, let me know and I'll send it by PM. Since I scheduled my audition (not via a taping) they sent me a soft-copy of the application. As a result I have the exact answers I submitted to them.
