Do y'all enter notes on every bill, or only in certain circumstances?

I always put something down.. usually where I got it from. Most of the time, I don't know where I'm going to spend it.....

Thanks. I entered some info about the condition of a few of the 19 ones I just entered. I'm not sure I'm diligent enough to enter the source every time.
