Bob's mother's 2nd cousin twice removed could be either her grandparent's sibling's great-great-grandchild, or her great-great-grandparent's sibling's grandchild. The kind of cousin is determined by the smaller number of generations you have to go back to get to the pair of siblings. The number of times removed is the difference between those numbers for the two cousins.

Because it isn't specified which cousin is closer to the siblings' generation, the final answer in BiT's example could equally have been 3rd cousins thrice removed, if that had been one of the choices.

I'm afraid I haven't explained it all that well. I hope you're less confused, anyhow.
