I presume the interactions with his staff in this episode really happened, but it would be easy to believe that it was all in his head until the moment he remembered everything and stumbled out of the bus. So, just before they get in that accident, she displays some obscure symptom of something that he just have to treat right now? What a season finale coincidence!

They weren't. As usual, they were probably arguing about Wilson. We'll find out next week, if she lives, which I'd put at 50/50. We have too many characters right now, and need to weed out a few. I'm in favor of any trimming that gets us more Cuddy.
