I had no idea about the answer, but I cry foul as the the phrasing of the question. Meredith mentioned, in passing, after the answer was revealed that this drink also contains cayenne pepper. Moreover, what she didn't mention was that all three of these ingredients are diluted in "pure water". For someone attempting to reason the question out, leaving out the water is not an insignificant matter, since the other three choices could imaginably be drunk straight up.

Furthermore, Wikipedia says that this drink isn't even all there is to it.
The cleanse involves drinking only a mixture of fresh lemon juice, grade B maple syrup and cayenne pepper, as well as a laxative tea and/or a saline solution to flush out the lower gastrointestinal system. No solid food is eaten for the entire cleanse.
That laxative tea might have more to do with the alleged "cleansing" than the maple lemonade.
